The Aesthetic Face

Profound RF and CORE Fusion Laser Resurfacing

Sound too good to be true? We thought so too at first. I spend countless hours every week studying the latest trends and offerings from the medical device market. Many devices come out on the market promising to cure this or fix that and believe me I have seen and heard them all! I am excited to share a device that truly delivers and that device is Profound RF.

So, you may have heard of radio-frequency and there are many treatments out there where a device is applied to the skin in multiple treatment promising to slim, tighten and tone the jawline and neck. Bottom line, multiple (often painful) treatments later the promised results do not arrive. Patients are often told at this point that they are “non-responders” and another treatment modality is offered, often surgery. You may also have heard of micro-needling, and this is a great treatment for superficial tone and texture of the skin! We often combine micro-needling and PRP to get a great superficial result. So what’s the problem with all this great stuff?? Micro-needling can’t generate the stimulation needed to create significant collagen, elastin and hyaluronic acid growth and radio-frequency on the skin cant penetrate deep enough to affect the layers of the face, jaw line and neck that create significant lifting and tightening.

PROFOUND RF!!! This device places electronic electrode pairs deep into the skin and then the pairs of electrodes emit radio-frequency heating the tissue to a specific scientifically validated temperature for a specific amount of time. This combination comes from clinical trials shown to generate optimum collagen, elastin and hyaluronic acid. So what does all this mean?? In 1-2 hours under local anesthesia you can come and undergo a non-surgical procedure that will lift, tighten, and slim (if indicated) the Face, Jawline and Neck!! The best part is that 100% of patients responded in clinical trials and on average patients received 37% the result of a facelift. If that sounds like something you are interested in the contact us today to schedule a Profound RF consultation at no charge.

Dr Green named Provider of the Week!

I am truly honored to share with everyone the recent news of being named provider of the week. As most of you know I serve as a residency associate program director in Portsmouth Va. While I truly love seeing and treating my patients in Newport News week in and week out i am also incredibly passionate about educating other surgeons and surgeons in training. In Portsmouth I perform the full scope of Facial Plastic and Reconstructive surgery as well as Head and Neck Cancer surgery. While sometimes this includes the removal of head and neck tumors, the majority of my practice rotates around Cosmetic and Reconstructive Facial Surgery. This ranges from microvascular flaps from the extremities to full facial rejuvenation. Every patient I meet and treat no matter if its microvascular flap reconstruction or a complex facelift procedures, is incredibly special to me and I am so grateful for this recognition. Thank you all!

Cannula injection techniques for facial fillers

Facial Fillers are an amazing adjunct to revitalizing the youthfulness of the aging face. As the face ages 2 things occur to the volume of the face 1) volume is both lost 2) volume descends due to gravity. Late aging often requires resuspension of the volume of the face using surgical lifting techniques but both early and late aging often benefit from “revolumization” of the face. This concept of facial aging can be thought of as the raisin theory. If you take a raisin and pull all of its wrinkles up skin tight what do you have… a tight raisin. What we want in facial aging is a full, luscious and beautiful grape again! We must get volume back!!

Fillers are one of the ways in which we regain volume. The other methods include facial implants and fat transfer which also are very successful especially for more significant facial aging! When it comes to fillers injection technique and filler material are everything! This video features the newest fillers from the Allergan line: Volbella and Vollure. These are two amazing fillers with excellent applications both on and off label. The addition of flexible cannulas really is a phenomenal addition to filler injection!

When it comes to technique, the use of a blunt tip flexible cannula truly is one of my favorites. My favorite cannula currently is the Dermasculpt Microcannula. The theory here is that the flexible blunt tip easily navigates through the dermis and subcutaneous tissues with lower rates of vascular injury and thus less bruising and inflammation. (Picture used from the Dermasculpt website click for redirection to their site). I truly hope you enjoy the video and as always feel free to email me with any questions or comments!!

This patient gratefully provided us an unedited 24 hr updated photo showing ZERO bruising and minimal swelling on the day following injections.

Replacing the Entire Lower Jaw: We did that!!

So I have been very fortunate in my life to work with some amazing people. The skill set I have been blessed with through my fellowships including Facial Plastic and Reconstructive Surgery includes microvascular procedures where we take one part of the body and move it to the head an neck and reconnect the blood vessels using a microscope. My two surgical passions are truly Facial Cosmetic Procedures and Complex microvascular reconstruction. The skills I have learned in these two fields may seem quite different but they complement each other in amazing ways. My reconstructive techniques only better help me appreciate the structures that give the face its form and function and thus optimize my Aesthetic outcomes. The Cosmetic Surgery principles I use every day get employed into my reconstructions and thus make reconstructive outcomes even better.

This article details an amazing case that I did with Dr Ramzey Tursun my friend and mentor in Microvascular reconstruction of the Head and neck. In this unique case report the entire jaw was removed and replaced with single bone from the leg. That new lower face was then fully rehabilitated with dental implants and teeth. To my knowledge this patient is the only such case fully restored with total microvasular jaw from condyle to condyle IN THE WHOLE WORLD of medical literature!! AACS 2018 here we come: Dr Green to speak with world renowned cosmetic surgeons

I am so very excited to announce an upcoming speaking engagement at the annual scientific meeting of the American Academy of Cosmetic Surgery in Las Vegas! It is always a privilege and a pleasure to be speaking with other cosmetic surgeons from around the world! This amazing event occurring February 1-3 in Las Vegas, Nv gathers the top Cosmetic and Plastic surgeons from around the world the discuss the latest trends in Aesthetic Medicine.

My topic this year is titled “Lifting more than the face: Optimizing outcomes through combined cosmetic procedures.” This is truly my passion. By comprehensively analyzing my patients faces and offering them an approach combining multiple cosmetic procedures in one surgical setting my patients get the results they want while minimizing their down time and eliminating the need for further surgeries. Facial aging is a complex problem and we shouldn’t pretend it can be fixed with one single procedure. It should be treated as the complex issue that it is and approached with a unique grouping of procedures to achieve each patient’s goals. Combing procedures is not only effective but safe and cost effective, eliminating the need for more time out of work or social life and the need for additional anesthetics.

I am so very excited to be speaking this year at a meeting featuring amazing cosmetic providers such as Dr Joe Niamtu, Dr Angelo Cuzalina,Dr Suzan Obagi , Dr Paul Nassif, Dr John Fezza, Dr Julie Woodward, Dr Sheila Barbarino, Dr Ryan Diepenbrock just to name a few!

Real Self™: The place to be for patient and surgeon info and interaction

Well the longer I do Facial Plastic Surgery and the more people I meet, the more I realize people LOVE to talk about cosmetic surgery!! It doesn’t matter If I am at the grocery store, on airplanes, at meetings, etc once people find out what I do they love to hear about it. I’ve also found out people love to talk about what to anticipate, what they experienced, or to ask questions when they don’t understand something.

So the first obvious question is where can all of this discussion happen. If i’d had that answer a few years back i’d be running a website instead of operating every day. I am kidding of course cause I don’t know what i’d do if I couldn’t operate, but seriously the people at Real Self™ have created an amazing forum where patients and professionals can interact in an open and accessible format. If you haven’t checked it out I would recommend it! Also, if you’re one of our patients and you’d like to tell other people about your experience in my care that would be a blessing as well! Thanks so much and enjoy!! Click the first link below if you’d like to visit my profile and the second link if you’d like to leave me a Real Self™ review! Thanks as always.
